0475(1) All staff informed the licensor either they would make the report to CPS if they had concerns or report to another staff member their concerns so they could make a report. For this reason, the violation is likely not valid. 0475 (2) During interviews, the program director and staff member Edith Woods stated to the licensor that they had performed physical restraint on the child before. This had not been reported to the department or the licensor. For this reason, this violation is likely valid.
